A randomized, double-blind, placebo-controlled study involving 30 participants with osteoarthritis (OA) investigated the effects of either a placebo or 5 grams of collagen peptides taken twice daily for 13 weeks. The results indicated that collagen peptides effectively improved OA symptoms. Researchers hypothesized that this effect may stem from collagen peptides’ ability to regulate chondrocyte differentiation and stimulate proteoglycan synthesis, thereby supporting cartilage tissue.

Further studies have examined the impact of collagen peptide supplementation on body composition, strength, and muscle health when combined with resistance exercise. In a randomized, double-blind, placebo-controlled trial with 57 recreationally active male participants, one group received 15 grams of collagen peptides while participating in a resistance training program. Both groups experienced a slight increase in strength; however, the supplemented group showed a more significant increase in fat-free mass. This suggests that collagen peptides enhance the effectiveness of physical activity. Muscle fiber biopsies revealed no notable differences between the test and placebo groups, prompting researchers to suggest that the increase in fat-free mass in the supplemented group was due to hypertrophy of contractile muscle cells and potentially increased connective tissue.

While many studies on collagen protein supplementation and resistance training have focused on male subjects, a separate 12-week randomized controlled trial involving 77 pre-menopausal women also indicated positive outcomes. The trial showed that those who combined 15 grams of collagen peptide supplementation with resistance exercise three times per week experienced improvements in muscle strength and fat-free mass.

Several systematic reviews and meta-analyses have strengthened the evidence supporting the effectiveness of beta-hydroxy-beta-methylbutyrate (HMB) supplementation on strength, body composition, and muscle health in both trained and untrained individuals. A meta-analysis of randomized, placebo-controlled trials—administering HMB in doses ranging from 1.5 to 6 grams per day over a period of three to nine weeks—found sufficient data to support strength gains in untrained participants, while the effects were negligible for trained individuals, showing little impact on body composition. Additionally, a meta-analysis reviewing over 250 different supplements for their benefits on lean mass and strength during resistance training identified only six with adequate research backing. Notably, nine studies on HMB were qualified for review, demonstrating that a 3-gram daily dose supported net gains in strength and lean mass.

A systematic review and meta-analysis examining HMB supplementation in a clinical setting involving 2,137 subjects with conditions characterized by loss of skeletal muscle mass and weakness aligned with earlier findings, suggesting that HMB supplementation can increase skeletal muscle mass and improve muscle strength.

Moreover, studies have investigated the combined effects of vitamin D and HMB. In a randomized, double-blind, placebo-controlled trial lasting 12 months with 117 older participants, HMB (3 grams) and vitamin D (2,000 IU) were shown to benefit muscular function, whether or not participants engaged in resistance training. Another study analyzed previous data on the vitamin D status of older adults using HMB supplements, revealing that muscle mass increased regardless of vitamin D status, but strength improvements were observed only in individuals with adequate vitamin D levels. This finding underscores a synergistic effect between HMB and vitamin D on muscle function.

Several systematic reviews and meta-analyses have helped to solidify the body of evidence supporting the effectiveness of HMB on strength, body composition, and muscle health in trained and untrained individuals.
